What causes it.
Yes. Two faults, both electrical, running at the same time. A barrel plug that only touches at one angle sparks against that contact every time it moves. An adapter on its last legs hands out dips and spikes where a steady twelve volts should be. All of that lands on the drive's board first, which is where the design intends it to land. The protection diode swallows what it can, then shorts itself and pulls the rail to earth. The board is finished and the discs behind it never hear about it.

The adapter goes on the bench too
Both were put under load. The adapter's output drifted and overshot the moment anything drew on it, which is what he had guessed. The drive's twelve-volt rail measured a hard short to earth, and next to it was the small sharp-edged burn a spike leaves behind. Age does not leave that mark.
Replace the parts, keep the chip
The burnt parts came off and were replaced, and the original ROM stayed exactly where it sat. Calibration figures for this one mechanism are held in that chip. Take them away and the electronics cannot interpret a thing the heads send up.
Clean power, then one run through
On a regulated supply the drive behaved as if the six weeks had never happened. It handed over its defect tables at the first attempt and crossed the whole disk at a level rate. That is how electrical faults tend to go. The damage stays in the electronics.
How it finished.
Nothing was missing. He tested it his own way: pick a year, list every order in it, count them, and the totals matched year for year going back. The adapter went in the bin. He got his answer in writing as well.
